I   agree   that   in   registering   myself   to   attend   the   Heal   America   Tour   event   that   I   will   hold   Urban Specialists,   Inc,  and Stand Together Chamber of Commerce, Inc. (collectively, the “Organizations”) and their  employees,   volunteers   and   any   other   persons   assisting   with   any   phase   of   such  event   harmless   from   any   and   all   liability,   claims,   and   responsibility   for   making   such   trip   and  activities.   I   further   release   all   of   these   parties   from   liability   by   reason   of   any   accident   or   injury that   might   occur   while   at   the   event   or   participating   in   any   such   activities.

I   hereby   grant   the   Organizations   the   irrevocable, transferrable,   right   and   permission   to   use   photographs   and/or video   recordings   of   me   on   Organizations   and   other   websites   on   the   internet   and   in   publications, promotional   flyers,   educational   materials,  advertising, publicity, or derivative   works,   or   for   any   other   similar   purpose without   compensation   to   me.    I   also   understand   and   agree   that   I   may   be   identified   by   name and/or   title   in   printed,   Internet   or   broadcast   information   that   might   accompany   the   photographs and/or   video   recordings   of   me.
